HD Hyundai Heavy Industries says changes made to the M/V Dali after delivery bypassed critical redundancies and triggered the second blackout that led to the fatal collision with Baltimore’s Francis Scott Key Bridge in March 2024. Responding to the NTSB investigation, the shipbuilder said the vessel was originally delivered with fully redundant power and fuel systems designed to automatically recover from outages. HHI alleges the ship’s operator replaced an automatic fuel supply pump with a manual flushing pump, eliminating redundancy and violating classification rules. According to both HHI and the NTSB, the modified system failed to restart after an initial blackout, starving generators of fuel and causing a second loss of propulsion and steering.
